Chairside vs Lab Milling: What You Are Actually Buying
There are two ways to bring milling in-house, and they price differently:
- Chairside, same-visit. A CEREC (Dentsply Sirona) system pairs a Primescan intraoral scanner with a Primemill wet mill to cut a lithium disilicate (e.max) crown while the patient waits. The Primemill runs roughly $45,000 to $60,000; the full chairside setup with scanner and a sintering furnace lands around $90,000 to $130,000.
- Lab or in-office lab mill. A 5-axis dry or wet mill handles zirconia, PMMA, and wax at volume. The Roland DWX-52DCi ($35,000 to $50,000, six-station disc changer), the vhf K5+ ($45,000 to $80,000), and the Amann Girrbach Ceramill Motion 2 ($40,000 to $70,000) are the workhorses.
Zirconia needs a sintering furnace (Ivoclar Programat, Amann Girrbach Ceramill Therm, or Dentsply inFire), $8,000 to $20,000. You will also want a desktop scanner (Medit, 3Shape) and design software (exocad, 3Shape Dental System). All in, a system runs $25,000 to $120,000. Killing the Lab Invoice: ROI and Section 179
The case for milling is arithmetic. An outsourced crown costs $150 to $400 in lab fees per unit. Milled in-house, the same crown is $15 to $40 of zirconia or lithium disilicate plus a few minutes of design and mill time. Say you send out 40 crowns a month at an average $220. That is $8,800 a month, roughly $105,000 a year, leaving the practice. Bring it in-house and most of that stays, while the note on an $88,000 system runs near $1,550 a month.
